Use these forms to request a deferment of your NDSL/Perkins Student Loans.
Electronic Funds Transfer (EFT) Authorization Form
Complete an authorization form to set up electronic funds transfers (including payment and deposits). Forms are available to be submitted online or download* a printable document to fill out. Please see Payroll Services' Frequently Asked Questions for payroll direct deposit information.
Request for Refund of General Property Deposit*
After leaving the University with no intention to return, complete a General Property Deposit form to request a refund or to donate your GPD to the Student Property Deposit Scholarship Fund.
Resident Tuition Waivers by Reason of Employment
